The Fishers Health Department is pleased to announce that we are now offering Sports Physicals to support our local student-athletes. As we continue our collaboration to serve the community, we want to ensure your students have a convenient and affordable option for their pre-participation evaluations. Service Details & Scheduling To better accommodate student schedules, we have dedicated specific blocks of time to maximize availability: • Primary Hours: Tuesdays and Thursdays | 2:30 PM – 6:15 PM • Cost: $25 flat fee (Please note: We do not bill through insurance). • Location: Fishers Health Department Clinic, 11400 Johnson Farm Way Fishers, IN Appointment Requirements To ensure a smooth experience for the athletes and staff, please keep the following in mind: • Documentation: The IHSAA physical form, the History Form (in green, pages 1&2) must be completed prior to the start of the appointment. • Parental Consent: Depending on the student's age, a parent or legal guardian may need to be present. • Vaccination Review: During the exam, our staff will review the student’s immunization records to ensure they are up to date with state requirements. While our appointment slots are limited, we are committed to capturing as many students as possible during our dedicated service hours.
